Using Unpleasant Symptoms Theory as a Guide
According to the research conducted in the fourth week, I will take the most important for the application of a tool for the evaluation of the patient’s unpleasant symptoms.
Although multiple tools exist to assess symptoms, few assess multidimensional symptoms. The unpleasant symptoms theory created and revised by Lenz, Suppe, Gift, Pugh, & Milligan, 1997, can be applied to nursing practice and research.
The TOUS indicates that symptoms could be measured according to intensity, time, discomfort, person to be evaluated, and qualitative dimension. At the same time, there are influencing factors, such as physiological history, age, sex, pathologies, pregnancy, surgery, medication, and radiotherapy. The factors are psychological, for example, the person’s mood, the treatment of the disease, and factors such as the environment, social aspects, and economic aspects. Furthermore, TOUS hypothesizes that failure to review, treat, or monitor these three dimensions would affect a person’s functional and cognitive performance (Peterson & Bredow, 2013).
  The implementation of this theory, is a guide for nurses and advanced nurses, beginning with obtaining clinical records that include physiological, psychological, and situational factors, for example, physiological symptoms would include consequences of illness, injury, or management. The psychological aspect includes identifying depression, mood swings, or trauma. Finally, identify the situational factors involved and identify living conditions, family problems, and concerns. (Peterson and Bredow, 2013)
  Likewise, the implementation of the theory allows evaluating results to periodically monitor changes in symptoms in the short and long term, giving instructions that inspire self-control and self-care. Lenz (2018)
Based on this, as a family nurse practitioner, I would seek to collect as much data as possible about the patient’s indicated symptoms. This evaluation tool must contain at least three aspects to evaluate: physical, psychological, and situational. Once these data were obtained, I would analyze their relationship and how one symptom could be related to the other. Then I would evaluate the priority with the patient and caregiver to implement a work plan that involves the patient, family, and nurse, and finally, I would periodically evaluate compliance with the objectives. It should be noted that this holistic work must require the support of other health professionals, such as case managers, social workers, psychologists, and psychiatrists, to fully meet the objectives.
