Create an electronic survey to distribute to students and families of your selected school. The survey should solicit information about the needs of students and families and their current access to community resources.
Include the following in the 15-20 question survey:

Three specific areas of focus (e.g., academic, medical, physical needs)
At least 3-5 specific needs families might experience within each area of focus
Current familiarity families have with organizations and resources that support each of the specific identified areas of focus
Level of comfort families have with sharing needs with school personnel
