You’ve set a SMART goal that has been approved.
You’ve submitted your Behavior Change Contract.
NOW -Beginning Monday, -September 13, 2021  start recording the number of minutes (or whatever variable) of your behavior change each day until the following Sunday. Do not make any effort to try to reach your first short term goal yet… this is just a chance to record your baseline behavior. (Next week, on Sept. 20, 2021 you’ll begin to strive for your first short term goal. )
Here’s How To Complete the Activity Logs:
Open the attached activity log template in Excel (see link above)
Save this template to your computer because you will add to it and turn it in for points every week for the next 9 weeks. 🙂
You’ll see page tab titles at the bottom left of the template. For this assignment, you will complete the daily PreChange sheet. Next week, you’ll go to the tab labeled Week 1, then Week 2, etc. 
Complete all sections for the particular week that is due including the minutes (or amounts) of your behavior, name, your SMART goal, 3 short-term goals, and minutes of the behavior for that particular week/day. 
PLEASE NOTE: Sample minutes have been applied to the template week to show you how it works. Please change these to reflect your progress. If you are measuring quantity rather than time, provide that info in the same spot and make sure to change the word from “Minutes” to “Amount” . Also, do not turn it in before the deadline so you can submit exact behavior.
 
Directions:
Turn in the Pre Change activity log you’ve been keeping by opening and filling in data into the Excel template I provided  at the beginning of Module 2. 
You’ll see page tabs/titles at the bottom left hand side of the template. For this assignment, you will select the tab/title of Pre Change. 
Complete all sections for the particular sheet  including your name, your SMART goal, 3 short term goals, and minutes/amount  of your stated goal behavior for that particular week/day. 
PLEASE NOTE: sample minutes have been applied to the PreChange week to show you how it works. Please change these to reflect your one approved goal and the minutes/amount of progress for that date.
